Take the first step towards receiving quality care services. Our experienced team will assess your needs and create a personalized care plan tailored specifically for you.
We begin with a friendly conversation to identify, understand your needs, and set the tone for trust and cooperation.
We gather personal and demographic details to understand your overall situation. Key information: Full name, date of birth, address, contact details, next of kin /emergency contact, cultural background, language, religion, and communication needs, living arrangements and support network, employment, education, social activities etc
Focus will be to gather all the necessary information and documentation to enable us fully to understand and assess your physical and mental health needs and how well you manage everyday tasks - Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
We will evaluate your living environment for safety and accessibility by assessing your home layout & accessibility (stairs, bathroom, lighting), fire safety, fall risks, hazards, assistive equipment needs (grab rails, walkers, etc.) and determine social and community resources available.
We shall explore and develop in your care plan strategies to counter factors affecting your emotional wellbeing and social inclusion: relationships and social support, hobbies, interests, and meaningful activities, spiritual or cultural.
We shall work with you to identify financial or legal issues that may impact your care. Evaluate your income, benefits, or pensions, explore funding eligibility (if applicable), consider power of Attorney or guardianship, and establish consent for sharing financial information
We start by summarising the assessment and identifying areas of need (e.g., mobility support, medication management). Note strengths and independence (what you can do yourself), and risks (falls, self-neglect, safeguarding) and thereafter discuss your goals and preferences for care. Following from this and in collaboration with you, we produce a PERSONALISED care plan based on the assessment to include, agreed goals and outcomes, specific interventions and support services, frequency and type of care visits, matched caregivers, agreed reviews and reassessment schedule. Before commencing care.
